Vaishnava Samhita. Narasimha Mehta Charitam I. Vol III Book VI Chp 15 Shlokas 21 - 23

शुद्धस्फटिकसङ्काशं नागयज्ञोपवीतिनम् |
पिनाकपाणिं लोकेशं चन्द्रशेखरमीश्वरं  ||
नीलग्रीवं विरूपाक्षं भस्मोद्धूलितविग्रहम् |
व्याघ्रचर्माम्बरं शरण्यं कारुनार्णवम्  ||
विलोक्य प्रेमसंमुग्धः पुलकाञ्चितविग्रहः |
परमाश्चर्यसंपन्नः पपात भुवि बालकः  ||

The young boy saw Shri Shiva as follows: Lord Shiva was lustrous like marble. He was wearing the snake as the sacred thread and was holding the trishul in hand. His locks were decorated with the moon's crescent. His neck was blue in color (under the influence of the poison consumed by Him). His eyes were beautiful with reddish tinch and He was clad in tiger-skin dress. Looking at Lord Shiva, the ocean of mercy, and one fond of the bhaktas, in this form, the child was surprised and became very happy. His hairs stood on end and he fell on earth.
